Description: Keep Well aims to increase the rate of health improvement in 45-64 year olds in areas of greatest need, with a particular focus on early intervention for those at a high risk of coronary heart disease and diabetes. NHS Health Scotland supports local NHS Boards in a variety of Keep Well programmes to engage and communicate effectively with the target audience.
