Summary: The Smoking, Health and Social Care (Scotland) Act 2005 came into effect in March 2006 and prohibited smoking in certain “wholly or substantially enclosed premises” including most workplaces. However, “designated rooms” in psychiatric hospitals and psychiatric units are exempt from the legislation by virtue of the Prohibition of Smoking in Certain Premises (Scotland) Regulations 2006.

The smoking rate amongst people with mental health problems is higher than that of the general population and there is also evidence to show relatively high rates of smoking amongst health professionals who work in mental health settings. Research commissioned by NHS Health Scotland reported much support for moving towards smoke-free environments within mental health services, and evidence from international literature and from case studies also suggested that this goal could be achieved.

Summary: The purpose of this paper is to initiate discussion on whether and how the recurring barriers to, and enablers of, effective delivery of health improvement identified through evaluations of national initiatives over recent years can be appropriately addressed.

This paper provides the findings from a thematic analysis of a selection of evaluation reports in order to identify what issues are commonly identified as impacting on the effectiveness of health improvement delivery.
