Final Report for the Independent Evaluation of HaHP

This report presents the integrated findings from a three-year evaluation that has utilised a range of approaches and methodologies. It focuses on key findings and the subsequent learning from these. Reference is made throughout to more detailed reports previously completed for specific aspects of this evaluation. The evaluation was conducted by a team of researchers based at the University of Glasgow and supported by grantholders from a range of Institutions.

Independent Evaluation of Community Projects
The Community strand of HaHP was subject to both external, independent evaluation and internal, project-based monitoring and evaluation. The following section gives

143 community projects were funded by HaHP, with a total of over 6000 participants, mostly based in disadvantaged areas.

Learning and Development
The Learning and Development element of Phase 1 of HaHP co-ordinated
the development of core learning opportunities and mechanisms so that
local people and professionals involved in the prevention and treatment
of CHD would be equipped with the appropriate knowledge, attitudes and
skills to fully participate in the partnership.

The Local Authority strand of HaHP was subject to both external, independent

Community Care
Community Care was one of four Renfrewshire Council associated projects. Through the 'HEAL' project, the aim was to improve the nutritional quality of food and food choices and to increase physical activity opportunities available to community care clients.

Health at Work
'Healthy at Work – Healthy for Life' was one of four Renfrewshire
Council associated projects. This project focused on Renfrewshire
Council being a major employer in Paisley, targeting the employees and
in particular manual workers with a series of health improvement
programmes.

Health Promoting Schools
This project involved the development of health education and promotion
activities across the pre-five, primary, special and secondary school
sectors within Paisley.

Healthercise
The 'Healthercise' programme aimed to encourage participation in indoor
and outdoor activities, particularly for people living in priority
areas with low activity levels.

NHS: learning outcomes


Developing Professional Links and Networks
One of four LHCC (Local Health Care Co-operative) associated projects, the aim
was to develop training opportunities and improve partnership working and links, so that staff would feel more confident broadening their roles.

Disease Register
This project involved the design, development and implementation of a technical CHD register (known as the Central Data Repository or CDR),
bringing together information from various other databases and patient record sources.

The register holds information about patients diagnosed with CHD or those at risk of developing CHD. It can be used to calculate patients' risk factors and to assist with clinical audit of CHD patients.

Improving the Patient Pathway
The aim of this LHCC associated project was to improve the patient pathway for patients with CHD by developing effective linkages leading
from the community, through the primary care/secondary care continuum and on into tertiary care where appropriate.

The purpose of this LHCC associated project was to maximise links between community-based services and primary care.

Secondary Prevention Supported by the Primary / Secondary Care Continuum
This project aimed to prevent or delay complications and further episodes of acute coronary syndromes in patients who had already had a diagnosis of CHD, so resulting in reduced morbidity and mortality in this group.

Risk Factors: learning outcomes

Diet and Nutrition
The aim of the Healthy Eating strategy was to support the continued development of an environment that supports healthy eating in order to assist the people of Paisley in developing healthy eating patterns and to ensure that consistent messages are given across a range of key settings.

Physical Activity
The physical activity strategy encompassed the NHS setting, the education sector, communities and workplaces. Work towards this was directed in two ways: firstly to make physical activity more accessible to everyone and secondly to increase physical activity in those who were currently inactive.

Tobacco
The Tobacco strategy group took a strategic approach to tackling tobacco issues to achieve cultural change within Paisley.

It aimed to provide support for those who expressed a desire to attempt to give up smoking and to ensure that appropriate and consistent messages were given across the range of settings with regard to both education and moving towards reducing exposure to environmental smoke.

Agenda Change

of HaHP, evaluating the extent to which Agenda Change was achieved in
the key agencies responsible for service delivery.

Capturing Change
HaHP was subject to a quasi-experimental survey designed to detect changes that might result from HaHP at the population level.

Inverclyde was chosen as a comparator site for Paisley and both were surveyed at the beginning of the project (March - September 2001) to capture a range of measures relating to demographics, general health, CHD-related health, CHD-related behaviours and risk factors, and other relevant information such as mental health and socio-economic influences on health. A follow-up survey was conducted in 2003 (October - November).

Partnership Working
The independent evaluation of HaHP consisted of four separate but linked approaches. These were: a quasi-experimental survey, the mapping
of the social context within which the project takes place, a theory-based approach; and a range of integrated case studies using
qualitative and quantitative methods.

Saturation
The main factors in determining whether saturation has occurred is: reach/coverage, intensity, adoption and dose.
