Have a Heart Paisley (HaHP)
The National Coronary Heart Disease Demonstration Project, Have a Heart Paisley, is a multi-agency, multi-sector partnership between the local community, primary and secondary care and the local authority.
Vision
Have a Heart Paisley's vision is for the people of
Paisley and future generations to have reduced heart disease, healthier, longer lives and hope for the future.
Phase1
Phase 1 of the project (2001-2004) has been evaluated by an independent research team based at Glasgow University and by an internal evaluation team. One of the key aims of the Heart Health Network is to disseminate the lessons from Have a Heart Paisley.

Phase 2

The Have a Heart Paisley CHD Register
One of the key strands of the project is the development of a CHD register that pulls together and interrogates data held in Acute Services, Primary Care, Community and the Health Board. This allows the identification of new or existing patients who are at elevated risk of heart disease and who require to have their current care adapted or a new care programme initiated.
