HaHP Phase 2: Secondary Prevention

HaHP's secondary prevention component focuses mainly on physical activity, including health coaching.

Overview

HaHP's secondary prevention component involves the expansion of the current British Heart Foundation pre-participation exercise screening and referral system, applying this to those who attend annual secondary prevention clinics in Primary Care.

Those who demonstrate a willingness to change their exercise behaviour will be referred to an Exercise Tolerance Test (ETT) service, then seen by a Health Coach who will refer to British Association of Cardiac Rehabilitation (BACR) / non BACR supervised exercise or other appropriate non-structured services.

Health Coaching

Health Coaching for secondary prevention is a method being tested on those with existing CHD, to provide one-to-one support and individualised guidance for lifestyle changes. It is seen as a set of skills that will be used with a person in order to guide positive health behaviour changes. It will be used specifically in relation to the CHD risk factor of physical activity but also incorporates raising confidence and optimism to aid the change process.
