HaHP Phase 2: Primary Prevention

Have a Heart Paisley's primary prevention work targets people in Paisley aged 45-60, with the intention of reducing that population's risk of cardiovascular disease.

Overview

HaHP's primary prevention activities are based around offering a CHD risk assessment health check to all those aged 45-60 in Paisley, and inviting those participants to take part in a "Health Coaching" programme to help them identify the health behaviour changes they wish to make and support them in making these changes.

At the start of Phase 2 of Have a Heart Paisley, a mapping of participatory needs assessment took place to assess the needs of Paisley people in relation to the three main CHD risk factors. In response to this, and using learning from Phase 1 of HaHP, activity has been focused on Tobacco, Healthy Eating and Physical Activity.

Data will be collected at various points during the coaching process to identify changes in health status and also behaviour change achieved pertaining to diet, level of physical activity, smoking status and wellbeing. In response to a mapping and needs assessment exercise, services relating to the three risk factors will also be implemented within Paisley.

Nurse Health Checks

Over the first few months of 2006, every 45-60 year old in Paisley received an invitation to have a free heart health check. The health checks are carried out by Have a Heart Paisley nurses, and take place at the individual’s choice of venue: their home, a local community centre or a primary care facility.

Checks take approximately 40 minutes and the assessment uses criteria such as blood pressure, cholesterol and smoking status to estimate the participant's risk of developing heart disease. The nurse is then able to advise clients on any action they should take to reduce that risk.

Health Coaching

Health coaching for primary prevention is a method being tested with 45-60 year olds at risk of heart disease to provide one-to-one support and individualised guidance for lifestyle changes. It is seen as a set of skills that will be used with a person in order to guide positive health behaviour changes. It will be used specifically in relation to the CHD risk factors of healthy eating, physical activity and tobacco but also incorporates raising confidence and optimism to aid the change process.

Health Coaching Induction Programme

Below are the various training materials which were used during the 'Have a Heart Paisley Health Coach Induction Programme' which took place in May 2006.

The programme was delivered to a team of 'Health Coaches', who offer one-to-one support to people in Paisley at risk of or with established CHD and consisted of training on a number of key topics.

Tobacco

The Tobacco strand of primary prevention work focuses on the following activities:

  • Give up and get going – An intervention to test out whether incorporating structured physical activity into a smoking cessation programme will increase cessation rates and adherence to programme (quit attempt).
  • Cutdown2quit – Using a harm reduction approach to reducing cigarette consumption with the final goal to quit.

Healthy Eating

The Healthy Eating strand of primary prevention activity focuses on the following activities:

  • Cooking on a budget courses – A 4-week course on how to cook healthily on a budget. People are invited to come along to a local college for 4x2.5 hour sessions. Chefs at the college run the classes, up to 15 participants at a time.
  • Fruit and vegetable home delivery – working with local suppliers to promote/signpost participants to the existing home delivery scheme offering a smaller £2.75 value box in order to enhance attractiveness for people with lower spending power).
  • Fruit and vegetable discount vouchers – Working with independent retailers and fruiterers within the most deprived ward areas to increase access and availability of fruit and veg via subsidy vouchers from HaHP. Clients receive £2 off fruit and vegetables on a weekly basis.
  • Weight management support – group and individual support to be offered with dietetic input.

Physical Activity

The Physical Activity strand of primary prevention activity focuses on the following activities:

  • Physical activity classes in conjunction with Renfrewshire leisure classes
  • Renfrewshire leisure "Go hard" discounted scheme links
  • Linking in with phase one community based dance and Tai chi classes
  • Give up and get Going – An intervention to test out whether incorporating structured physical activity into a smoking cessation programme will increase smoking cessation rates and adherence to programme (quit attempt)
