Publication name: Evaluability assessment of community hub pilots in NHS Fife and NHS Forth Valley
Date: 30 August 2017
Description: Scottish Government has in recent years increased work to look at new models of working in primary care. This has included the development of Community Hub Pilots and GP Community Hub Fellowships (GP Fellowships) in Scotland for two pilot areas. This paper presents an evaluability assessment of this work to inform decisions about monitoring and evaluation of the pilots.

The report outlines the context for the development of the pilot sites, a theory of change and the key outcomes expected, an assessment of the data quality and availability for those outcomes, options for monitoring and evaluating the Community Hub Pilots and an outline of recommendations for evaluation.
