Publication name: Evaluation of the Glasgow 'Deep End' Links Worker Programme

Description: The Links Worker Programme is a Scottish Government funded programme piloted in deprived areas of Glasgow. It aims to support general practice patients with complex needs to live well through strengthening connections between community resources and primary care and to mitigate the impact of the social determinants of health. The University of Glasgow conducted a three-year, mixed-method, independent evaluation of the Links Worker Programme, commissioned by NHS Health Scotland. The final report sets out the main findings and recommendations from the evaluation.
