Publication name: Optimising Older People's Quality of Life: an Outcomes Framework – individual models

Description: The Scottish Government launched the Reshaping Care for Older People (RCOP) programme in 2011, supported by a £300m Change Fund, with the aim of encouraging health and social care services to move towards a more preventative approach. The outcomes framework for this strategy was developed by the Scottish Government’s Integration & Reshaping Care policy team, the Joint Improvement Team (JIT) and NHS Health Scotland and is made up of two main components: a strategic outcomes model and four nested logic models that illustrate a range of preventive services areas. Ultimately, the outcomes framework will be transferred to the JIT website as an online resource that can be further developed and easily updated. The written version of the framework is an interim step.
